When your account was created, you were assigned a username and password.

You also provided an email address that is associated with your account.

If you forget:

• Password:  Click on the "password" link below the login fields...you will need to provide your username AND email address, click submit, and an email will be sent that contains your NEW password
• Username:  Click on the "username" link below the login fields...you will need to provide your email address and an email will be sent to you containing your username
